Berberine for metabolic health

Who it's for

Adults with creeping fasting glucose, HbA1c in the 5.7–6.4% pre-diabetic range, or elevated triglycerides — looking for an evidence-graded complement to diet, walking after meals and resistance training. Not a replacement for metformin or any prescribed therapy.

What the evidence says

Multiple RCTs and meta-analyses (most around 500 mg, 2–3×/day) show berberine lowers fasting glucose by ~0.7–1.0 mmol/L, HbA1c by ~0.7%, and improves total cholesterol and triglycerides on the order of statin-light effects. Evidence grade: B for glucose and lipids — consistent human trials, mostly in Asian populations, modest sample sizes.

How to take it

500 mg with each main meal (2–3×/day). Take with food to reduce GI upset. Cycle 8–12 weeks on, 2–4 weeks off, and re-check fasting glucose / lipids with your GP. Berberine has poor oral bioavailability — splitting doses across meals matters more than total dose.

Safety and interactions

Berberine inhibits CYP3A4 and P-glycoprotein — it can raise blood levels of statins, cyclosporine, certain blood pressure meds and anticoagulants. Do not combine with metformin without medical supervision (additive hypoglycemia risk). Avoid in pregnancy and breastfeeding.
